Have you ever had your licence revoked?: Please answer yes or no truthfully, this information will also be found on the SIA database.
What is the full licence number shown on your SIA Licence Card? Please ensure you fill in the full, correct SIA licence number. This is the 16 digit number on the front of your card (see example below). If you have more than one SIA Licence Card please enter ALL licence numbers here with a space in between them.
What is the expiry date?: Please fill in the expiry date of your SIA Licence, making sure you use the calendar function and it is in the DD/MM/YYYY format.
